Your Role
You will:
- Provide evidence-based care to patients in their homes or other care settings
- Develop and implement individualized treatment plans
- Work closely with patients and their families to help them achieve their goals
- Collaborate with other healthcare professionals to deliver comprehensive care
- Document patient progress and outcomes accurately
- Stay current with the latest evidence-based practices
- Contribute to the development of new programs and initiatives
